Dr. Jacob Chwiedz

Assistant Clinical Director – Physical Therapist
PT, DPT

I’m a first generation Polish-American that grew up in Colchester, Connecticut, where I played baseball until I attended university at UConn. With my decision to focus on studies, along with a fresh perspective and newfound freedom, I felt a drive to continue pushing myself physically at the gym and began learning everything I could about exercise and nutrition. From then on, there was no question that my bachelor’s degree would be in Exercise Science. But I didn’t stop there. I knew I wanted to help people live their best lives and take care of something fundamental to doing that–their physical health–a challenge everyone has to deal with. I believed I had the ability to do just that by pursuing a Doctorate in Physical Therapy. Needless to say, I’m happy where I am today and look forward to a career I would be hard-pressed to trade for any other.

Experience:

  • Bachelor’s of Exercise Science (UConn ’17)
  • Doctorate of Physical Therapy (UConn ’20)
  • 5 years of clinical experience in outpatient orthopedics
  • Years working as a Therapy Aide and Rehab Tech

Core values

  • Being humble.
  • Always growing as a person.
  • Aiming at the highest good.

“Ever since I was a child, I have had this instinctive urge for expansion and growth. To me, the function and duty of a quality human being is the sincere and honest development of one’s potential.” – Bruce Lee

Patients have to deal with the challenge of keeping themselves healthy for a lifetime. I want to be a reason they get healthy and stay healthy the rest of their lives so they can live life with less limitations.

I’m the Assistant Director for our Wallingford/Meriden location as well as a full-time treating physical therapist assisting with the company’s efforts in providing the best patient care, and supporting each and every employee that make that possible.

I particularly enjoy working with the lumbar spine as that is uniquely something I have had to deal with growing up as a baseball player.

I’m happy to be part of an excellent clinical team that is striving together to set a standard for Physical Therapy and truly making a difference in patient’s lives.

Patient Testimonials

They are the first therapists that actually did a comprehensive history on me. My Orthopedic history is vast and Jacob listened and adjusted my therapy to my needs. Emily is wonderful too, friendly and attentive and prompt. Thank you for all your great care.

Very personalized individual care. Both John the receptionist and Jacob my therapist were very kind, caring and supportive. I have had physical therapy at other places and usually for the standard cookie cutter exercises for that condition. At [Blank] Physical Therapy Jacob really studied my body movements and came up with some very helpful ways to move and stand to dramatically ease my pain. Some were very simple and I had immediate improvement. All of the movements and exercises were tailored for my issue and much different than anything I have experienced in the past. I would highly recommend this practice.

The expertise of Jacob and his sensitive manner along with Emily’s efficient ways and smiling face make my visit to their office a pleasant and medically worthwhile experience.
